Report Blames Pilot in F-22 CrashSays pilot shut down oxygen systemAn air force accident investigation board report blames the pilot in a 2010 crash of an F-22 Raptor in the Alaska wilderness. The AIB report confirms that the pilot, Air Force Captain Jeff Haney, caused the F-22 Raptor crash by shutting down the oxygen system. The AIB faults the pilot for not reacting quickly enough to activate the jet’s emergency oxygen system or recover from a dive he entered into as he struggled to regain his air supply. The report raised questions among critics about the Air Force’s frequent blaming of pilots for accidents when there are hardware and software malfunctions. The crash happened south of the Denali Highway near Cantwell during a nighttime test mission in November of 2010.

Procedures are pounded into pilots to allow rapid recovery from problems, they may have concluded he did not react fast enough to the failure.....but it was still a system failure. This is not a failure of an indicator in a 172 flying 95kts where you have lots of time to think. This failure caused the loss of a brave warrior and true hero to our country.
