Daycare Where Toddler Died Reopened
One-year-old’s death ruled an accident

ANCHORAGE - Investigators ruled the death of one-year-old Faith Phillips an accident and Illuminations Daycare, where she died, is back up and running.

Thursday, the Wasilla Police Department, which worked with the U.S. Consumer Product Safety Commission and the State Childcare Program Officer, released the investigation report after a six-week investigation.

According to the report, an Illuminations Daycare worker found the one-year-old unconscious at 10:54 a.m., with her head stuck in what's described as a u-shaped hole in a piece of playground equipment called “The Castle.”

According to the investigation, another worker was nearby sweeping, but didn't notice the toddler hanging by her neck.

Police said surveillance footage shows Faith alive 13 minutes before she was found unconscious. But investigators said the castle was in a blind spot, behind another playhouse.

The surveillance camera didn't cover the part of the castle where Faith apparently suffocated.
